CC4C (Care Coordination for Children)
Services for children 0-5 with Medicaid and certain risk criteria.
Care manager works with the family to meet their needs. Goals of
program are to reduce number of hospital admissions and emergency
department visits, quicker referral to other services and increase
number of children with a medical home.

Smiles Dental Project
Provides dental treatmentfor children birth to 5years of age who qualify.Provides dental education,supplies, and referral support for all pre-schools.
Immunizations
Immunizations provided
according to current State Immunizations Recommendations.
Limited immunizations for travel
for a fee.
Walk-in clinic for immunizations every Friday 8:30am-4:30pm. Other
days by appointment.
School Health
Early identification of special health needs
through teacher referral and routine screening by a Public Health
School Nurse. Educational outreach and other initiatives as designated by
school health policy.
